Location: Third floor of the University Health Service Building
UHC Travel Health Services are available to the following:
- Enrolled students
- Faculty and staff and their dependents ages 10 and up
- Individuals who have a UHC primary care provider (PCP)
Costs:
- Travel consultations: free for enrolled students; $100 for faculty, staff, alumni, and non-students
- Vaccinations and prescriptions will be billed to your insurance. If your insurance does not cover the costs, you will be responsible for payment. Out-of-pocket costs for vaccinations are available here.
- Over-the-counter medications and other supplies may be recommended for purchase.
- Is your department paying for some or all travel costs? If so, you will need to bring documentation from your department including their shortcode for billing. Any charges not covered by your department will be your responsibility.
Steps:
- Register your travel.
- Call UHC or send a message in your patient portal to request a travel questionnaire.
- Complete and submit your questionnaire.
- You will need a copy of your vaccination and/or titer records for reference to complete the questionnaire.
- Check your insurance coverage to determine what vaccines your plan may cover. Out-of-pocket costs for vaccinations are available here.
- After your completed questionnaire is submitted and reviewed, you will receive a message to self-schedule your appointment.
- Enroll in U-M’s Travel Abroad Health Insurance. All U-M students, faculty and staff traveling abroad for University of Michigan Related Travel are required to have this insurance. It is also available for students, faculty and staff on leisure travel abroad.
